After many wonderful years I have retired from the practice of medicine. I will greatly miss the many wonderful patients that I’ve had the honor of working with over that time.
MEDICAL RECORDS
Most patients will not need a copy of their records at this time.
The easiest process is to have your records requested by your new provider. This will be done electronically. Our Fax number remains 757-410-7395.
For now, the requests are still being handled by our office, but in a few months, this will be done by the new custodian of the records. More details will be provided as this becomes active.
If you need a copy of your records more immediately, a HIPAA-compliant release form must be signed and filled out. This should be submitted to us via your Athena Patient Portal (also linked here), fax 757-410-7395, or by US mail. Our phone number 757-410-7390 will remain active also.
Note: We can not respond to email or voicemail medical records requests.
